Registration Forms

At Family Medical Group, we provide a full range of medical care including:

General adult medicine
Office gynecology (Annual check ups, contraception)
Pediatric and newborn care
Evaluation and treatment of skin conditions
Splinting and casting of fractures
On-site x-ray and EKG

Our physicians also perform minor office surgical procedures (removal of skin cancers, vasectomies) and will assist the primary surgeon of our patients whenever possible for those procedures occurring at Emanuel Medical Center.

Directions
Our office is located at 911 E. Tuolumne Rd., between Olive and Colorado, in the city of Turlock.

Hours
We are open from 8:00 a.m. to 5:00 p.m. The office closes from 12:00 - 1:00 p.m. for lunch. In the event of an urgent problem such as severe pain or sudden high fever, we will do all we can to see you or your family member the same day. If this is not possible, our Urgent Care Center is available to care for your urgent medical needs.

Appointments
Our receptionists will be happy to assist you in making an appointment from 8:00 am - 12:00 p.m. and from 1:00 p.m. to 5:00 p.m. We do our best to accommodate your schedule based on the availability of our physicians.

We do require a cancellation notice of 24 hours.

Emergencies
When an emergency occurs, please contact the office with as much detailed information as possible. If your doctor is not available, we will direct the information to the doctor on-call. If the emergency occurs after hours, the answering service will forward the information to the doctor on call for the group. He will return your call as quickly as possible. Some situations cannot wait for a returned phone call. In these cases, call "911" for an ambulance or go immediately to the nearest emergency room.

Night and Weekend Call
A doctor will always be available to address problems which truly cannot wait until the office opens. Our answering service is able to contact the on-call physician. Weekend call is shared with several area Family Practitioners, so you may speak with a doctor who is not part of our group but still is very qualified to provide care and answers to your questions.
